Why We Built This

Vancomycin is one of the most common antibiotics used in hospitals, and also one of the most difficult to dose correctly. It has a narrow therapeutic index, significant inter-patient pharmacokinetic variability, and the consequences of under- or over-dosing are serious: therapeutic failure in one direction, nephrotoxicity in the other.

The 2020 joint guidelines from the American Society of Health-System Pharmacists (ASHP), Infectious Diseases Society of America (IDSA), and Pediatric Infectious Diseases Society (PIDS) changed the standard of care by shifting from trough-only monitoring to AUC-based dosing. We built this calculator to make those guidelines accessible to every clinician, not just those with access to expensive Bayesian dosing software.

This tool calculates loading dose, maintenance dose, dosing interval, estimated AUC₂₄, elimination half-life, and estimated trough from six patient inputs, using the same pharmacokinetic equations described in Rybak MJ et al. Pharmacotherapy 2020;40(13):1280–1315.
